I know very little of the
technicalities of Italian politics, as our media generally seems to prefer to report on only the more salacious details, typical of trends in the media worldwide.
Well, I was aware that you believe were two different persons,
and for that my assessment.
Usually he is referred as Prime Minister because its TV networks have been forced to call him "premier" which is the contraction of "Prime Minister" by himself, and this make confusion, even for the foreign media. Hence, note how for a politician to posses TV networks/press agencies can manipulate the reality. As for the political details:

Nobody elect him directly as Prime Minister but people elect the Parliament;
- The Parliament vote to "
propose" its candidate (without the approval of the oppositions, of course);
- The candidate prepare the Council of the Ministers;
- The Executive is addressed to the President of the Rupublic which "control" the right things and then
grant the men of their functions (the President of the Council
cannot sovrimpose its will above the President of Republic);

The Parliament can discourage the action of the Executive and to call for the President of Republic in
revoking their functions;
- The President of the Republic split the Parliament and proclaim new elections (
of the Parliament, not of the Executive)
- Repeat the steps. Thats it.
Therefore, there are some points for which Mr. B. cannot be equipared to a Prime Minister.
